Independent Contractor Documents
- Complete Independent Contractor Packet This packet contains all of the forms necessary (or potentially necessary) to be set up as an Independent Contractor (IC) to serve RCO's customers. Completing this packet alone does not qualify you to work. Be sure to wait for the customer and the Personal Agent to tell you that you are cleared to work.
- Independent Contractor Statement This form confirms that you believe you meet the definition of “Independent Contractor” as defined in the Oregon Revised Statutes. Also included is a copy of the ORS relating to Independent Contractors.
- Mandatory Abuse Reporting Notice This provides information on abuse and what to do if you suspect it.
- Change of Information Form This form is used to notify RCO of changes to either employer or employee information, including address and phone number. Use when necessary and send to the RCO office.
- IRS Form W-9 This form is required by the IRS for RCO to report earnings.
